The forms have a long history and have been tested in the churches for many years. They serve as guiding texts for the administration of the sacraments and other ecclesiastical acts.

Child Baptism

Form for the administration of baptism to adults who have come to faith.

The full form for the celebration of the Holy Communion.

Shortened version of the Communion form for regular use.

Communion – Modern Afrikaans

The Communion form translated into modern Afrikaans.

John Calvin's original Communion form.

Other

Confession of Faith

Form for the public confession of faith.

Excommunication

Form for the excommunication of a member from the congregation.

Readmission

Form for the readmission of an excommunicated person to the congregation.

Ordination of Ministers

Form for the ordination of ministers of the Word.

Ordination of Elders and Deacons

Combined form for the ordination of elders and deacons.

Ordination of Elders

Form for the ordination of elders into office.

Marriage Form

Form for the solemnization of marriage before God and the congregation.
