The forms have a long history and have been tested in the churches for many years. They serve as guiding texts for the administration of the sacraments and other ecclesiastical acts.

Child Baptism
Form for the administration of baptism to children of believers.
Adult Baptism
Form for the administration of baptism to adults who have come to faith.
Communion
The full form for the celebration of the Holy Communion.
Communion – Shortened
Shortened version of the Communion form for regular use.
Communion – Modern Afrikaans
The Communion form translated into modern Afrikaans.
Communion – Calvin
John Calvin's original Communion form.
